About MED Suppliers Network

The MED Suppliers Network is a group of durable medical equipment suppliers who have met the quality standards and accreditation requirements set forth by CMS and were awarded contracts for Competitively Bid items. Also listed on the site are durable medical equipment suppliers across the nation who can supply product to Medicare patients who are not currently affected by Competitive Bidding.

About the Equipment Request Form

The equipment request form is a tool created for the healthcare provider (home health agency, physician office, sleep lab, discharge planner, case manager or social worker) to complete a minimum amount of information to order specified equipment from a Medicare DME Supplier in order to facilitate the patient receiving their equipment in a timely manner. The end result is an easy to use document to be printed and faxed to the selected Durable Medical Equipment supplier to begin the order process.
